Policies:
• Cancellation Policy: there are no refunds if an applicant cancels a registration
• We kindly ask families to refrain from bringing food and drinks into the gym and to turn off all cell phones
• Please arrange to have your child picked up on time
• A Fun 'N' Fit enrollment card must be completed on or before the first day of class by a parent or legal guardian
• Unfortunately we cannot accommodate any make-ups

Waiver & Consent: I agree to let my child * participate in tennis. I understand there are certain risks of injury in the participation of this sport and am willing to assume these risks. I confirm that my child is capable of participating in tennis and that he/she is in good physical condition. In addition to giving full consent to my child's participation, I waive, release and hold harmless Gotham City Tennis, LLC and the Reebok Sports Club/NY, Ltd, its officers, coaches and representatives for any injury that may be suffered by my child.

In addition, I understand and agree that Gotham City Tennis, LLC retains the right to expel my child from its programs if he/she engages in conduct that is dangerous or disruptive to its programs. I further agree to let Gotham City Tennis, LLC use any images or pictures of my child in the program for advertising or marketing purposes.
