Gotham Tennis Academy offers school vacation mini-camps for all ages and levels at Stadium Tennis Center at Mill Pond.

You may sign up for a single day or any combination of days that fit into your plans.

I understand and acknowledge that the risk of injury is inherent in any program involving physical activity. As the Parent/Guardian of *, I hereby waive and release any and all full rights and claims for damages I may have against NY Tennis at Mill Pond, LLC, Gotham City Tennis, LLC, its principals, agents, staff and tennis professionals, for any and all injuries sustained by the player. This includes transportation to and from the program in connection with any participation in this program.

In addition, I understand and agree that NY Tennis at Mill Pond and Gotham City Tennis, LLC retain the right to expel my child from programs if he/she engages in conduct that is dangerous or disruptive to the programs. I further agree to let NY Tennis at Mill Pond, LLC and Gotham City Tennis, LLC use any images or pictures of my child in the program for advertising or marketing purposes.
