Please remember that your child’s participation in the Gotham Tennis Academy Program is a standing weekly commitment which cannot be rescheduled, except in the event of a cancellation by Gotham Tennis. Credits for cancellations by customer are issued at Gotham Tennis Academy’s sole discretion. No refunds. Note: There is a 3% additional processing fee applied to all credit card payments.

Waiver & Consent: I agree to let my child * participate in tennis. I understand there are certain risks of injury in the participation of this sport and am willing to assume these risks. I confirm that my child is capable of participating in tennis and that he/she is in good physical condition. In addition to giving full consent to my child's participation, I waive, release and hold harmless Gotham City Tennis, LLC and the Dalton School, its officers, coaches and representatives for any injury that may be suffered by my child.

In addition, I understand and agree that Gotham City Tennis, LLC retains the right to expel my child from its programs if he/she engages in conduct that is dangerous or disruptive to its programs. I further agree to let Gotham City Tennis, LLC use any images or pictures of my child in the program for advertising or marketing purposes.
